My best advice is to inform the waitstaff that you (obviously) have a toddler with you and would like to get through the meal as quickly as possible. Then you tend to get a little more attention and no waiters purposely stretching out courses or going on their 15 minute breaks without checking on you. I also ask for little bowls of cheese or fruit right away to keep them occupied. That's made all the difference for us!
